This is an example of my a template I designed in Excel. This work is from a retirement community that had not update any of their public documents in almost 7-10 years.  The meal ticket (pictured below) was used to either record a special meal delivery or placed on a table to reserve it for guests.

The original document was lost so new copies were made using the best blank copy that could be found.
